29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed: Extrusion department, portable unknown/unmarked sheet shear: On or about January 2, 2018, the employer did not inspect hazardous energy control (lockout) procedures periodically (at least once per year) to ensure it was adequate and being followed in accordance with the requirements of 29 CFR 1910.147. Operators assigned to perform setup tasks (servicing) in the point-of-operation and associated danger zones had their fingers and hands exposed to unexpected cutter start-up likely to result in amputation injury when accessing the shear point to pull material through. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
