29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork: On or about December 7, 2017, Sloan Implement Company, Inc. had an employee exposed to approximate 11 foot fall hazards, while working from a supported scaffold (parts box), which was not secured to the forklift by a chain or some other secure method.
