(i) Except as provided elsewhere in this section, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a) Facility, 7100 Mellon Road, Export, PA: On or about November 21, 2017, an employee worked on an upper landing surface that was 8' 4" above the concrete floor without any method of fall protection in place.
