29 CFR 1910.147(c)(8): Lockout or tagout was not performed only by the authorized employees who are performing the servicing or maintenance: (a) Site: In the production area, Small Erema, 5 Otis Street, Westborough, MA 01581 on or about 11/22/2017 and at times earlier the Small Erema was locked out by a LOTO authorized employee and the servicing or maintenance associated with the lock out was performed by a different employee (a machine operator) who was not a LOTO authorized employee. (b) Site: In the facility production area, Line 11, 5 Otis Street, Westborough, MA 01581 on 2/12/2018 and at times earlier the Automatic Scrap Regrinder (ASR) was locked out by a LOTO authorized employee and the servicing or maintenance associated with the lock out was performed by a different employee (a machine operator) who was not a LOTO authorized employee.
