29 CFR 1926.960(c)(2)(i): The employer did not ensure that the employee, when working within the employer's established minimum approach distance, also uses rubber insulating sleeves when an employee uses rubber insulating gloves as insulation from energized parts. On or about 9/19/17, at the overhead power lines across the street from 3279 Elliswood Street, Sulphur, LA; the employer allowed an employee to connect the field phase wire near an energized jumper line without ensuring that the employee also continued to wear rubber-insulating sleeves.
