29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: a) Plant, Light Foam Area - On or about 03/08/16 and at times prior thereto employees applied lockout/tagout during maintenance/repairs of the Bun Foaming Machine. Only one person applied locks, while the rest of the workers were unprotected from the sudden start of machinery and potential errors during shift changes. The employer did not modify the existing procedure with instructions for the protection of employees performing maintenance as a crew. HP Pelzer Automotive Systems, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1910.147(f)(3)(i), which was contained in OSHA inspection number 1133070, citation number 1, item number 2-a, issued on 09/07/2016 and affirmed as a final order on 09/21/2016, with respect to a workplace located at 1884 Warrenton Hwy, Thomson, GA 30824. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
