29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used: a) Leitner-Poma of America, Inc., 1599C Summit County Road, Base of Peak 10-Falcon Super Chair, Breckenridge, CO 80424: On or before July 20, 2017 the employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used. Employees were working on a catwalk that was 12'9" high to disassemble a two-person chairlift. The employees were not trained on which personal fall arrest system to use, how to anchor to maintain 100% fall protection, how to connect to limit the radius of a fall, and how to maintain and inspect the personal fall arrest system used. This condition exposed employees to fall hazards of 12'9".
