29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Chicago Die Cutting Press 770: On or about May 10, 2017, lockout or tagout devices had not been affixed to each energy source of the press by the authorized employees prior to engaging in die removal and die setting process. This exposed the press operator to crushing hazard and amputation injury.
