Grainger Inc. - Mezzanine area: Authorized Employees conducting maintenance and unjamming operations for conveyor machinery were not trained on Lock Out / Tag Out. The employer did not ensure that authorized employees were trained on all aspects of the energy control program and all specific procedures for machinery such as conveyors, occurred on or about 02/02/2017.
