29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees: (a) Servtech at 333 Centennial Pkwy. Louisville, CO: On or before 1/19/2017, the employer did not ensure that employees required to use machines with multiple energy sources, such as, but not limited to the Machine Direction Orient Drill-Line CNC hole cutting machine (Ocean Machinery, Model: MDL-1000/1C, Custom Avenger 40, SN: A002943), were given specific lockout tagout training or guidance. Employees were therefore not protected form hazardous energy. This condition exposed employees to amputation hazards.
