29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. Big Ox Energy - Siouxland, LLC - 1617 D Ave - Dakota City, NE 68731 - The employer is failing to ensure that employee(s) are wearing appropriate eye and face protection from hazards when it is necessary for employee exposure to reactive, corrosive liquid chemicals. On or about January 8, 2017, two employees, wearing inadequate ppe, suffered face, scal, neck, arm, and body burns when they were exposed to a violent chemical reaction while pouring 34% hydrogen peroxide and VTX chemical catalyst down a facility floor drain.
