29 CFR 1910.335(a)(2)(i): Employees did not use insulated tools or handling equipment that might make contact with such conductors or parts when working near exposed energized conductors or circuit parts. At the job site, an employee was using a fiberglass duct rod with an attached conductive copper wire near an open energized pad-mount transformer exposing the employee to electric shock hazards.
