29 CFR 1910.147(e)(1): Before lockout tagout devices were removed, the work area was not inspected to ensure that nonessential items had been removed or ensured that machine or equipment components were operationally intact. a) Palram 2000, Inc., P3 - On or about December 1, 2016, after knife blade changing operations were completed on the Bryer Extrusion Lines brand D-78224 cross cutting saw the machine knife carriage metal mesh guards were not replaced prior to re-starting the machine.
