29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. a) Throughout Facility - On or about February 9, 2017 and at times prior to, authorized employees cleaned equipment and did not affix a lockout device to energy isolating device(s) prior to starting service and maintenance, thus exposing employees to caught-in, pinch point, and crushed-by hazards.
