29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: a) Perdue Farms, Milford DE - On or about November 3, 2016, the employer failed to ensure that where lockout devices were not applied and employees were engaged in servicing and maintenance of the Gizzard Machine, each employee was removed from the work area before energy was restored to the equipment, exposing employees to amputation hazards created by the chain and sprocket.
