29 CFR 1910.147(c)(7)(i)(B): Each affected employee shall be instructed in the purpose and use of the energy control procedure. a. Located at 100 East Broadway Street in Jackson, Ohio: On or about August 21, 2015 and at times prior, employees were performing lockout on Extruder 1 located in the Fernandez Processing Plant without being trained on the energy control procedure, exposing employees to an amputation hazard. b. Located at 100 East Broadway Street in Jackson, Ohio: On or about August 21, 2015 and at times prior, employees were performing lockout on Extruder 15 located in the Goodwin Processing Plant without being trained on the energy control procedure, exposing employees to an amputation hazard. c.
