Flexible cords were not connected to devices and fittings so that strain relief was provided which will prevent pull from being directly transmitted to joints or terminal screws. (a) At the Pia Marine Hotel project, the outside insulation on an extension cord was not connected to the plug that was used to power two portable Ridgid cut off saw and Makita square sander, exposing employees to the hazard of contacting live electrical wires.
