Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to the employee in that: On or about 3/23/93 employees were exposed to the hazard of being caught and/or crushed between the elevator and shaftway wall or sills. Car gates were not provided on the North and South sides of the elevator car. Among other methods one feasible and acceptable abatement method to correct this hazard is to provide the elevator car with a gate or door at each entrance equipped with a car door or gate electric contact.
