29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Dry Grinding Areas, Factory Floor, EP Heller Company, 21-25 Samson Avenue, Madison, NJ 07940. The employer failed to provide medical evaluations for workers exposed to carbide steel dust and required to wear Uline brand N95 filtering face pieces during dry grinding operations. Condition observed on or about 10/8/2014.
