29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: a) 1st Floor - On or prior to October 3, 2014, the "elevator" (dumb waiter) doorway area did not have a standard railing in place exposing employees to fall hazards of approximately 10 feet. b) 2nd Floor - On or prior to October 3, 2014, the "elevator" (dumb waiter) doorway area did not have a standard railing in place exposing employees to fall hazards of approximately 20 feet. c) 3rd Floor (Attic) - On or prior to October 3, 2014, the "elevator" (dumb waiter) doorway area did not have a standard railing in place exposing employees to fall hazards of approximately 30 feet.
