29 CFR 1910.178(l)(1)(ii): Prior to permitting an employee to operate a powered industrial truck (except for training purposes), the employer did not ensure that each operator had successfully completed the training required by 29 CFR 1910.178(l): a) Prior to permitting an employee to operate a powered industrial truck, the employer does not ensure that each operator has been trained. This violation occurred on or about September 18, 2014, and at times prior thereto, at a workplace located at 350 W. Heritage Drive, Tyler, Texas; where employees operated a rough-terrain forklift truck without having first completed training consisting of formal instruction, practical training, and an evaluation of the operator's performance in the workplace.
