29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: a) 125 Factory Ln, Middlesex, NJ site - Process area: Employer failed to establish a definite time interval between inspections or tests for pressure relief valves at the worksite based on operating experiences, good engineering practices and manufacturers' recommendations. Condition noted on or about 02/05/15. RAGAGEP such as but not limited to API 510 which gives guidance as to establishing and implementing defined inspection intervals was not followed: 6.6.2 Testing and Inspection Intervals 6.6.2.2 Unless documented experience and/or a RBI assessment indicates that a longer interval is acceptable, test and inspection intervals for pressure-relieving devices in typical process services should not exceed: a. Five years for typical process services; and, b. Ten years for clean (non-fouling) and noncorrosive services.
