29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): a) A & D Wood Products, Inc., Elida, Ohio: On or about August 26, 2014, the employer did not ensure that a pulley used to drive the resaw feed conveyor was guarded. Employees operating the swing saw, the resaw, and others working in the area were exposed to crushing and amputation hazards from the ingoing nip point created by the belt and pulley system. b) A & D Wood Products, Inc., Elida, Ohio: On or about September 5, 2014, the employer did not ensure that three drive pulleys equipped on the MMT manufactured notching saw were guarded. Employees engaged in saw dust cleanup activities were exposed to crushing and amputation hazards created by ingoing nip points from the belt and pulley system.
