29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: a) Drop out box of EAF exhaust system: On or about June 2, 2014 and at times prior thereto, the employer did not evaluate the need for, select, provide and require use of respiratory protection to protect employees from the thermal and chemical hazards associated with inhaling hot EAF dust that becomes airborne while conducting the contracted services.
