29 CFR 1926.503(c): The employer did not ensure that: When the employer has reason to believe that any affected employee who has already been trained in fall protection requirements of 1926.503(a) does not have the understanding and skill required by paragraph (a) of this section the employer shall retrain each employee. Circumstances requiring retraining include but are not limited to situations where there are, (1) changes in the workplace, (2) changes in types of fall protection systems or equipment, (3) employees understanding or skill level indicates the need for retraining: Employees were not provied with additional training on fall protection procedures to use when working at the top deck during concrete hirise building construction. a) Employees were exposed to 50 ft fall hazard when installing plywood decking, formwork, and guardrails at the outer edges of the top deck. b) Column openings were left uncovered in the plywood top deck, awaiting column forms to be placed into the holes by crane. 15 employees were exposed to 10 ft fall hazard at (3) deck openings which were 3 to 4 ft wide. Location: 81 Fleet Place, Brooklyn, N.Y; hirise concrete construction worksite. On or about 2/24/2014 NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM IN ACCORDANCE WITH 29 CFR 1903.19.
