29 CFR 1926.502(i)(3): All covers were not secured when installed so as to prevent accidental displacement by the wind, equipment, or employee: The employer does not ensure that all floor hole covers were secured to prevent accidental displacement by employees. This violation occurred on or about November 18, 2013, on level 4 at a workplace located at 3521 N. Plano Road, Richardson, TX 75082, where a hole cover was not secured when installed so as to prevent accidental displacement and exposed at least one employee to the hazard of falling to the lower level.
