29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section: (a) The open sided stairs to the cross over of the slab transfer line on the east side did not have a handrail on the north side next to the transfer chain; (b) The open sided stairs to the cross over of the slab transfer line on the east side did not have a midral on the south side of the stairs; (c) The open sided stairs to the cross over on the slab transfer line on the west side were missing midrails on both sides of the stairs; (d) The open sided stairs to the cross over of the cant transfer chain line on the east side did not have handrails on either side of the stairs; (e) The open sided stairs to the cross over of the cant transfer chain line on the west side did not have handrails on either side of the stairs; (f) The open sided stairs to the head saw operator's entrance walkway did not have any handrail on the south side; and (g) The open sided stairs to the head saw operator's entrance walkway did not have a midrail on the north side.
