29 CFR 1910.132(e): The employer did not ensure that employees were not using defective or damaged personal protective equipment in the workplace: (a) On or about September 4, 2013, for an employee who fell approximately 55 feet from a tubing board during a well servicing operation while wearing a defective cable positioning lanyard that was tied to the tubing board at SW/SE, Sec 24, T153N, R102W, south of Trenton two miles east of Highway 1804 South near Trenton, North Dakota.
