29 CFR 1910.134(f)(2): The employer did not ensure that an employee using a tight-fitting facepiece respirator was fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter: (a) Piezo Kinetics, Inc. - The employer did not ensure that an employee wearing disposable respirators was fit tested prior to initial use of the respirators, on or about September 13, 2013. (b) Piezo Kinetics, Inc. - The employer did not ensure that employees wearing half mask respirators were fit tested on an annual basis, on or about July 19, 2013. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
