29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (a) On or about August 27, 2013 at the 700 E. Jones Street, Milwaukee, Wisconsin Preliminary Treatment Building, all energy sources for the Exhaust Fan 7-1 were not locked, blocked, and tagged to render all moving parts inoperative. An employee was exposed to amputation hazards from a moving belt.
