29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 23 August, 2013 and at times prior to, at the worksite located at or near 1405 Pace Road, Pocahontas Arkansas, the employer did not ensure the employees were protected from multiple energy sources during maintenance by procedures for an energy control program to include items such as: a. Shut down procedures for the rotary injection machine did not outline the scope, authorization, and techniques for control and dissipation of energy sources. b. No procedural steps for placement, removal and transfer of lockout or tag out. c. No requirements for testing the rotary injection machine to determine effectiveness of energy control measures.
