29 CFR 1910.95(i)(2)(ii)(A): The employer did not ensure that hearing protectors are worn by any employee who is exposed to an 8-hour time-weighted average of 85 decibels or greater and has not yet had a baseline audiogram established pursuant to 29 CFR 1910.95(g)(5)(ii): (a) On or about 8/23/13 at Salvione Enterprise, for employees working the band mill. The employer did not enforce the use of hearing protection where employees were exposed to over the action level for noise. An employee feeding the mill was exposed to 87.6 dBA of noise in 428 minutes, where the 8 hour action level for noise is 85 dBA. Zero exposure was considered for the remaining 52 minutes.
