29 CFR 1910.135(a)(1): The employer did not ensure that each affected employee wear a protective helmet when working in areas where there is a potential for injury to the head from falling objects: (a) Throughout Facility - Employees working with suspended materials using a sling and other related lifting equipment and were exposed to head injuries were not wearing protective helmets, on or about 5/28/13.
