29 CFR 1910.119(h)(3)(ii): The contract employer did not assure that each contract employee was instructed in the known potential fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan. On April 17, 2013 or at times prior thereto at the E-1 exchangers, in the Catalytic Hydro-Desulfurization Unit - 2 (CHD-2) at ExxonMobil Beaumont Refinery the contract employer did not assure that each contract employee was instructed in the known potential fire and explosion hazards of flame cutting the flange bolts on the channel heads of the E-1 exchangers.
