29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that employees acquired the knowledge and skills required for the safe application, usage and removal of energy control devices On or about March 22, 2013 and at times prior thereto, the employer did not train all authorized and affected employees in the recognition of all energy sources, the purpose and use of a lockout/tag-out system and the general knowledge about the hazards of unexpected release of energy. Employees performing maintenance work on the equipment such as but not limited to the Niagara metal press, and the Dayton 1 ton overhead crane were exposed to electrical shock and amputation hazards.
