29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. On or about February 22, 2013, the employer did not ensure the tubular welded frame scaffold located on the back side of building was equipped with guardrails and end frames. The employees working on the scaffold were exposed to fall hazards of approximately 13 feet to the ground below.
