29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) Beaver Builders and Excavation, Inc. @ 4251 Grove St., Denver, CO 80211: On 1/16/13, the employer did not ensure that a trench, which measured approximately 8 ft. x 3 ft. x 8 ft. deep, was inspected by a competent person prior to the start of work. This condition exposed an employee to struck-by hazards.
