29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed: In the Warehouse at this establishment, employees operate electric equipment in the performance of daily activities and the electric panels and boxes energizing the equipment had unused openings exposing employees to electric shock such as but not limited to: a. On the south wall adjacent to the overhead door, the Siemens electric panel, 120/208V, 200 Amps, had a unused opening on the bottom right of the panel. b. On the south wall adjacent to the overhead door, the metal light switch had a unused opening on the bottom of the box and the live conductors were visible. c. On the south wall adjacent to the exit door, the Siemens electric panel, 120/208V, 200 Amps, had two unused openings on the bottom of the panel at each end. d. On the east wall for the Baler, the Square D Safety Switch, 30 Amp, 600VAC, 250 VDS, 3 Phase, had an unused opening on the bottom of the box.
