29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person: 16 Dog Lane, Storrs, CT 06268: On or about 12/27/2012 the employer did not utilize a qualified person to design a scaffold to acount for the additional wind load emposed by adding a wind screens to the scaffold. A fabricated metal frame scaffold collapsed due to wind forces applied to the wind screens.
