29 CFR 1910.23(a)(2): Ladder way floor opening(s) or platform(s) were not guarded by a standard railing with standard toeboard(s) on all exposed sides (except at the entrance to opening) with passage through the railing either provided with a swinging gate or so offset that a person could walk directly into the opening: (a) Bulk Mixing Mezzanine: Platform ladder way opening used to access work platform for adding pigment to the aggregate mixing drum, did not have the open passage way guarded with a swing gate or offset, exposing employees to a seven (7') foot ten (10") inch fall hazard. (b) Outside Aggregate Hopper Work Platform: Platform ladder way opening used to access work platform did not have the open passage way guarded with a swing gate or offset, exposing employees to an eight (8') foot fall hazard.
