29 CFR 1910.304(a)(2) A grounded conductor was attached to a terminal or lead so as to reverse designated polarity. It was found on October 12, 2012, the employer designed, produced, and installed unapproved equipment. Through the development and manufacturing process of the Smart Box 3, the design of the casing for the smart box did not account for the power boot to be turned 180 degrees on the vertical, resulting in switching the neutral and hot conductors for the system. No safety testing had been done once a complete Smart Box 3 unit was created to determine if the completed, end user piece of equipment would be safe for employees to use.
