29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) On or about October 11, 2012, and at times prior to that date, for the employees installing siding from an Alum-a-Pole Pump Jack Scaffold System without adequate fall protection systems or adequate bracing in place, located at 600 Second Avenue SW in Watford City, North Dakota.
