29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: a) At the work site and on September 5, 2012, the employer failed to adequately instruct employees in the recognition and avoidance of unsafe conditions in that employees were not familiar with the manufacturer's safety instructions contained in the Operator's Manual for a John Deere 690 E LC Excavator.
