Text For Citation: 01 Item/Group: 001B Hazard: LOCKOUT
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to fire, explosion and steam hazards: (a) Boiler room; There was no documented and implemented quality control system for the three "Titusville" automatically fired natural gas boilers that were converted from oil to natural gas. The control panels were altered with "Wagner" lockout boxes. Equipment and parts such as, the main fuel gas regulators, welding on the steam piping system, repaired combustion chambers, adjusted damper arm linkage for air/fuel ratios were not documented on a quality control and inspection program. Among other methods, one feasible and acceptable means of abatement is to implement a documented quality control program for materials, parts, repairs and inspections as outlined in accordance with API 510.
