29 CFR 1910.178(m)(5)(i): When a powered industrial truck is left unattended, load engaging means shall be fully lowered, controls shall be neutralized, power shall be shut off, and brakes set. Wheels shall be blocked if the truck is parked on an incline. a) On or about December 30, 2025, at an establishment located at 200 Summer Street in Clintonville, Wisconsin, outside Building 3; employees were exposed to struck-by hazards when an operator got off their forklift and went inside the building. The load engaging means were not fully lowered when the operator got off the vehicle. b) On or about December 30, 2025, at an establishment located at 200 Summer Street in Clintonville, Wisconsin, in Building 3; employees were exposed to struck-by hazards when an operator got off their forklift after the unsecured load fell off the forks.
