29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. At the establishment located at 1695 Mills Street, Green Bay, Wisconsin; the employer did not provide one or more methods of machine guarding to protect the operator and other employees in the machine area in the following instances: a) On or about and at times prior to March 20, 2026, the Ribbon Blender #1 (Mfr: APV Crepaco, Inc, Model: DR, SN: K-1560) was not adequately guarded to prevent employee access during operation, exposing employees to contact with rotating parts. b) On or about and at times prior to March 20, 2026, the Ribbon Blender #2 (Mfr: APV Crepaco, Inc, Model: FB, SN: K-1561) was not adequately guarded to prevent employee access during operation, exposing employees to contact with rotating parts. c) On or about and at times prior to March 20, 2026, the Ribbon Blender #3 (Mfr: APV Crepaco, Inc, Model: FB, SN: K 1563) was not adequately guarded to prevent employee access during operation, exposing employees to contact with rotating parts. d) On or about and at times prior to March 20, 2026, the Ribbon Blender #4 (Mfr: APV Crepaco, Inc, Model: FB, SN: K-1562) was not adequately guarded to prevent employee access during operation, exposing employees to contact with rotating parts. e) On or about and at times prior to February 11, 2026, the Twin Screw was not adequately guarded to prevent employee access during operation, exposing employees to rotating parts. f) On or about and at times prior to February 11, 2026, the in-running nip point on the Barrel Conveyor 3 was not adequately guarded during operation to prevent entry into the machine, exposing employees to caught-in and entanglement hazards. g) On or about and at times prior to February 11, 2026, the in-running nip point on the Barrel Conveyor 2 was not adequately guarded during operation to prevent entry into the machine, exposing employees to caught-in and entanglement hazards. h) On or about and at times prior to February 11, 2026, the in-running nip point on the Line Attendant Rework Main Conveyor was not adequately guarded during operation to prevent entry into the machine, exposing employees to caught-in and entanglement hazards. i) On or about and at times prior to February 11, 2026, the in-running nip point on the Line Attendant Rework Backup Conveyor was not adequately guarded during operation to prevent entry into the machine, exposing employees to caught-in and entanglement hazards.
