29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a. On or about November 18, 2025, at the finishing area location at the assembly line of the facility employees were exposed caught-in and struck-by hazards when they performed maintenance and servicing on the machines such as, but not limited to the Solder Robot Machine, SMMS-14575, finishing line. The employer did not ensure that the authorized employee was knowledgeable on the type and magnitude of the energy to be controlled, the hazards of the energy to be controlled and the means to control the energy prior to performing the maintenance work [per the 1910.147(d)(1) requirements]. The employer failed to ensure that the energy control application steps were implemented as the machine was not shut down or turned off to perform the servicing work [per the 1910.147 (d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)], and verification of isolation [(d)(6)], were not implemented to protect the employees from the Solder Robot Machine, SMMS-14575, finishing line servicing hazards. The condition exposed employees were exposed to caught-in hazards.
