29 CFR 1910.333(b)(2): The circuits energizing the parts were not locked out or tagged or both in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): a.) Ellwood City Forge Company; 800 Commercial Avenue Ellwood City, PA 16117, on or about 08/24/2025, and times prior thereto: Employees were working inside the established working distance of the energized General Electric 4160 volts T2/T3 switchgear in the Millwright Shop Room. The line side of the 4160 volts switchgear was not de-energized or locked out and/or tagged out prior to performing work operations. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.
