29 CFR 1910.134(d)(2)(i):The employer did not provide either a full facepiece pressure demand SCBA certified by NIOSH for a minimum service life of thirty minutes or a combination full facepiece pressure demand supplied-air respirator (SAR) with auxiliary self-contained air supply: At a worksite located at 2630 McClelland Boulevard, Joplin, MO 64804: Employees were exposed to asphyxiation and thermal burn hazards. This was demonstrated when employees entering a walk-in freezer where large quantities of dry ice were being stored were exposed to unsafe levels of carbon dioxide gas. The employer did not provide respiratory protection of a full facepiece pressure demand SCBA certified by NIOSH for a minimum service life of thirty minutes or a combination full facepiece pressure demand supplied-air respirator (SAR) with auxiliary self-contained air supply with the use of dry ice that created a situation that was immediately dangerous to life and health (IDLH).
