29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person: On or about May 28, 2025, employees performing roofing work were exposed to fall hazards of over 6 feet and the employer did not ensure the Guardian Temper roof peak anchors were installed under the supervision of a qualified person: a. Two employees working on the south-facing roof area were attached to a single anchor designed for a maximum of one (1) personal fall arrest system. b. Two employees working on the south-facing roof area were attached to a single anchor secured to the roof with only one nail on each side.
